About This Role
We are seeking a compassionate and organized Patient Liaison with home health experience to support daily office operations and deliver an outstanding patient experience. In this role, you will work closely with patients, families, referral sources, intake, scheduling, and clinical staff to coordinate care, resolve concerns, and ensure smooth communication throughout the patient journey. Our Patient Liaisons play a vital role in creating a positive patient experience by building strong relationships, providing exceptional communication, and ensuring patients feel supported throughout their care journey. The Patient Liaison will be reporting to the Office Administrator. This position is based in our Florida offices, serving both the Boca Raton and Pompano Beach locations.
Key Responsibilities
- •Support daily administrative operations for the home health department
- •Assist with patient intake, scheduling, and communication
- •Build positive relationships with patients, families and referral partners
- •Resolve patient concerns and coordinate with internal departments
- •Maintain accurate records and ensure HIPAA compliance
- •Provide general office and administrative support
- •Assist with insurance verification and scheduling processes
