This level of assistance is ideal for patients who retain some mobility but whose balance, strength, or confidence has been diminished by illness, surgery, or deconditioning.

What to Expect

Your caregiver will provide verbal cues and standby support during transfers between bed, chair, wheelchair, and toilet. They ensure assistive devices are used correctly and walking paths are clear of obstacles.

Staying Safe During Transfers

Always use your assistive device as prescribed. Lock wheelchair brakes before transferring. Wear non-slip footwear. Take your time and avoid rushing, especially at night.

When to Contact Your Care Team

Contact your care team if you experience increased difficulty with transfers, if you have a fall or near-fall, or if you need more assistance than standby support. Sudden changes in mobility should be evaluated promptly.
