Understanding Patient and caregiver education

Patient and caregiver education is woven into every skilled nursing visit, but dedicated education sessions allow nurses to focus on building the knowledge and confidence patients and families need to manage care safely between visits. Effective education reduces anxiety, improves adherence to the care plan, and empowers families to recognize problems early.

Nurses tailor education to each patient's learning style, health literacy level, and specific care needs. They use teach-back techniques to verify understanding and provide written materials that caregivers can reference at home.

Topics Covered in Education Sessions

Sessions may cover disease-specific self-management, medication administration techniques, wound or device care, nutrition and hydration, fall prevention, recognizing emergency warning signs, and when to call the nurse versus calling 911.

Making Education Stick

Ask questions during every visit — no concern is too small. Practice new skills with your nurse present before doing them independently. Keep written instructions in an accessible location. Involve all caregivers in education sessions so everyone shares a consistent understanding of the care plan.

When to Contact Your Care Team

Contact your care team if you feel unsure about any aspect of the care plan, if you or a caregiver need a refresher on a procedure, or if the patient's condition changes in ways that make current instructions unclear. Never hesitate to ask for clarification — patient safety depends on understanding.
