Understanding Cognitive retraining

Cognitive retraining addresses deficits in attention, memory, problem-solving, organization, and executive function that result from stroke, brain injury, or neurological disease. These cognitive skills underlie nearly every daily activity, from managing medications to making safe decisions about cooking and driving.

Speech-language pathologists use structured exercises and real-world task practice to rebuild cognitive skills or teach compensatory strategies when full recovery is not expected. The goal is to maximize the patient's ability to function independently and safely.

What Cognitive Retraining Involves

Therapy includes attention exercises, memory drills, problem-solving tasks, sequencing activities, and real-world application of cognitive strategies. Therapists also train patients to use external aids — such as calendars, checklists, and smartphone reminders — as cognitive supports.

Supporting Cognitive Recovery

Complete homework exercises between therapy sessions. Get adequate sleep, as fatigue significantly affects cognitive function. Minimize distractions during tasks that require concentration. Use organizational tools consistently and keep them in the same location.

When to Contact Your Care Team

Contact your therapist if you notice sudden worsening of memory, attention, or problem-solving abilities. Report increased confusion, difficulty following previously manageable routines, or safety concerns related to cognitive function. Seek emergency care for sudden confusion, disorientation, or neurological changes.
